I haven't paid attention to mail standards in a long time.  Is RFC 2231
a happening thing?  Do we need to go through the trouble of supporting it?

I've seen it used - you'll have to deal with it every time you get an attachment
that has a non-ASCII name.  However, in the first pass you can probably mostly
ignore it - just don't write code that prohibits implementing it later.
