Nope. My mts.conf is pretty plain, except for masquerading.
Mail is submitted to a local instance of sendmail.

I do 'moderate' customization (AUTH mechanisims, envelope masquerading,
additional mailer definitions on different ports in order to send mail
to both my personal ISP's SMTP server and the SMTP server at $WORK,
customized smarttables to choose the SMTP server based on the recipient
address).
I haven't used nmh to actually do mail submission to anything other than a
local instance of sendmail for at least 10 years...as I recall, nmh didn't
offer enough customization, and I needed a local sendmail daemon anyway
for other mail purposes, so why maintain two very different methods of
submitting mail?
My guess is that, within the extremely small (and vocal, and dedicated,
and talented) pool of nmh users, most people are using nmh to submit mail
to a local mail server. While mts.conf and the whole nmh mail submission
system are in need of attention, I do wonder about the level of effort
that will take vs. the benefit it would bring. Of course, I'm not doing
any coding on nmh, so it's not my itch to scratch.
Another question for your survey might be:
3) If you are using a back-end agent for mail submission, and
enhancements to nmh provided more options & control over
mail submission, would you do mail submission directly via
nmh or continue your current practice?
