I believe this can in fact happen with legitimate mailings when
inexperienced users employ certain mailing systems by specifying a
distribution list in Bcc: and nothing in To:, so that the
Apparently-to:'s get generated without the inexperienced user's
knowledge or intention. Therefore, somebody processing incoming
mail might want to allow for this situation. (This very issue comes
up from time to time on comp.mail.pine: "How did all those
Apparently-to:'s get there???? I didn't put them there!")
