Anti-SPAM recommendations

This topic is covered in the FAQ's "Related Links", but I was hoping the
list could indulge me with a more subjective response: what is the best
combo of anti-spam procmail recipes available that does a reasonable job
without requiring a lot of updates and hand-tweaking?

It appears that some tools require constant maintenance because they filter
based on always-changing blacklists. Others require that you keep a list of
mailing lists and other accepted senders in order to allow that traffic to
pass (this is a moving target for me and something I want to avoid). Most
preferable is a ruleset that followed general hueristics where possible,
with easily-edited secondary blacklists that did not depend on continual
updates.
