Occasionally?  Probably a mail server burp, or even possibly a rule problem 
at your end (c flag).  Make sure you're not actually getting a CC: on a 
message you've replied to.

It's not Dan. I have them; the have different (and length) lists of Received; 
headers.

Note they have different message-ids (and the headers are capitalised 
differently). In contrast, my messages have message-ids created here
