It doesn't. Spamassassin doesn't have that capability. It only labels spam.
Acting on that label is the job of another program, such as Procmail. However
the required_hits config option sets the leve at which Spamassassin determines
an item is spam. When an item is determined to be spam by SA, it sets the
X-Spam-Flag header to YES. This is a binary flag, it's either YES or NO. This
is much easier to check in Procmail than complicated regexps for counting
stars or compring X-Spam-Level.

the global local.cf is set for 5.0, the 15.0 in the personal settings won't
apply AFAIK...

No, the user's config overrides the global, at least for that setting.
