Procmail is what I've posted. Nothing special is setup in Sendmail. Would
you like to see my M4 file?
When the lines:-

/dev/null
#almost-certainly-spam
Were like:-

* ^X-Spam-Level: \*\*\*\*\*\*\*\*\*\*\*\*\*\*\*
almost-certainly-spam
Those emails were still ending up in the users inbox as well as
almost-certainly-spam. Now that they are going to /dev/null they are going
to neither. Maybe I should update my procmailrc config to:-
:0:
* ^X-Spam-Level: \*\*\*\*\*\*\*\*\*\*\*\*\*\*\*
/dev/null
#almost-certainly-spam
:0:
* ^X-Spam-Status: Yes
probably-spam
:0:
* ^X-Spam-Status: Yes
/dev/null

Spam is put in probably-spam, but it's also going to the users
mailbox. It's definitely going though procmailrc as it's got he
[SPAM] header and report added by spamassassin. So I guess it's
coming back to SendMail which is using Virtusertable to deliver the
mail to the user.
Then your MTA (sendmail) is configured very oddly, or your procmail
is not what you pasted.
