As a system administrator, regardless of the OS and individual programs in
use, LOGS are your friend and you should become VERY familiar with the
logging options and output of EVERY piece of software you deal with.

None of the people you ask the question of can answer you because it is
happening on YOUR system to which we have no access.

Pick an email that generated a bounce when it shouldn't have. Follow that
email through the logs of every program it hit along the way starting with
the original smtpd handshake upon initial receipt of the message. 

If your logs are tuned down to save drive space then turn them up for a few
hours. If there's a user generating "hundreds of replies" that should be
long enough to catch one for tracking.

You will need (as a start) your MTA and procmail logs.
