Once your reputation is trashed or your domain is blacklisted, how do
you get that fixed?

I doubt that you'll get blacklisted due to a spammer every month or two.  
That would block too many white-hat ISPs that are indeed willing to combat 
spamming users.  But in case you're really burrowed in black tape up to 
your ears, you may try to get away by buying accreditation.  Which will of 
course get pretty expensive over time unless you really manage to better 
yourself.
