Why?

The helo + nohelo example:  A policy can set one of these flags, and
redirect to another policy without the mutually exclusive flag.  If that
other policy later adds the mutually exclusive flag the effect is
unclear.

If that's the case, then it is a bug in the specification of the "helo/ 
nohelo" option.

All the "op=" main spec should do about all this is advise designers of new 
options to take care of specifying all the possible odd cases.

A strategy "each modifier (or option) defines its own rules" could result
in rare and hard to debug errors.

Well, that depends on the quality of individual new options' specifica- 
tions.
