You need to have the MSHelp namespace declared in your stylesheet to use
that form.
So, you might do:
<xsl:stylesheet xmlns:xsl="..."
xmlns:MSHelp="whatever-the-right-namespace-is">
...
<xsl:element name="MSHelp:keyword"/>
</xsl:stylesheet>
