I think Mike means that your stylesheet must start with <xsl:stylesheet>
otherwise it is not XSLT because it is not even XML. This is the basics.
You may want to look up a tutorial. A first start is here:
http://www.w3schools.com/xsl. You can find some more pointers here:
http://en.wikipedia.org/wiki/Xslt (look at the bottom)
