Ah, but the text:span pushing is different ... I'm doing an explicit push of those elements. Perhaps the problem is not that the key isn't working, but that the template rule isn't even being engaged.

Did you confirm that the <xsl:otherwise> processing is found in the output? If not, that would be another clue.
