If you are generating a text file, use xsl:output method="text" then you
don't need disable-output-escaping (it does nothing in that case)
If you are generating an XML file, then you want to generate the element
node with name hd1 rather than a start tag <hd1> and the XSLt serialiser
will automatically output the right thing.
