Yes, in response to my report at the time, the eXist developers revised
the processing to optimize by reading the predicate early. And they did it
quickly, in time to make it into an addendum to the presentation at slide
#54.

expression like preceding::word[1]. That's really painful. I do hope they

(Yes, the problem here is different than in the paper, and here keys
are likely to be the best solution. But nice to know you're not alone
with this sort of issue.)
