I can enumerate three types:

You enumerated theoretical possibilities. There are many ways to generate
multiple address fields. Shellscripts can be made to do any legal or
illegal behavior. They are not packaged user agents.
However the request was for the names of existing UAs that do this today.
The question is about user agent products (commercial or non-commercial).
