  Unless there is an enhancement I don't know about, MH does not support
authenticated SMTP.

  I suggest installing a local MTA (exim, postfix, sendmail) that supports
authenticated SMTP, if in fact that is what your ISP really wants. I'd check
with your ISP to be sure about this. Ask them if you can do TLS authenticated
relaying instead of username/password.

  This is also a win if you aren't always online - MH won't spool messages
if your ISP is down.
