Under the current standard, rfc822, comments and folding white-space
are allowed before and after all lexical tokens in a message-id.
The internet draft of the revision of rfc822 specifies that software MUST
accept such message-ids, but MUST NOT generate message-ids that contain
comments or (unquoted) white-space between the angle brackets. Comments
and folding whitespace are still allowed outside of angle brackets.
So, the answers to your questions are, respectively, yes, yes, and
parentheses.
