The system was running qmail and procmail.
The simple fix was to modify the 1 and only line in
/var/qmail/alias/.qmail-local-default
FROM:
| forward $DEFAULT(_at_)foo01(_dot_)foo(_dot_)com
TO:
| forward $DEFAULT(_at_)foo01(_dot_)foo(_dot_)com foocopy(_at_)foobar(_dot_)com
Now all incoming and outgoing email is blind copied automatically to
foocopy(_at_)foobar(_dot_)com

I'm attempting to create automatic logging at an offsite address, e.g. to
meet a legal requirement, without having to rely on a CC or BCC field that
might be dropped by the user.
