Greg Connor's explanation was quite clear for me (thanks again Greg). 

I have been bouncing back and forth between different software products
i.e. messagewall, maildrop, spamassassin, and as in my case, it was
easy to misunderstand the implementation between MAIL FROM and
mail FROM or mail from for that matter. 

Adding an explanation to an area that might be misunderstood is an
excellent way to point the new user/implementor in the right direction
up front, and lends itself to better documentation.

Here is a quick conversation I had with my mailer.  I made the MAIL-FROM 
different from the From: address in the message to see how it came out in 
the logs and on the message.
