I'm that single user xyzzy (a vanity subdomain). In that
case it's no per-user policy, only a wildcard solution.
publishing %{l} is too high of a risk that the reputation
of the entire domain be tarnished by one single user
If that user publishes +all and spams the world his provider
will kick him and maybe publish -all for some time until the
spam zombies run dry and use another MAIL FROM.

after they've considered all the implications?
With a working abuse desk it's "only" a support nightmare.
You won't find it for free accounts, it's a premium service.
