But that's not correct.  Permerror is only for _real_ errors,
not for violating a SHOULD.   Some of these SHOULD didn't
exist in older versions, and so old policies might not know
about the SHOULD.  There can be rare cases where adding ?all
would run into size problems.  In other words if you see a
SHOULD then it doesn't mean MUST, otherwise the spec. would
say MUST.

One point of Permerror is that an erroneous policy should get
a Permerror from all conforming implementations.  It would be
bad if it sometimes gets a NEUTRAL (for default ?all) and at
other places a Permerror (for the missing ?all).
