The <b> tag is still around, but not being used as you might think:

IMHO, adding extra tags purely to achieve visual effects is as bad as old html tag 
soup with table-layout cream. Ok, it's fun & tempting to play with CSS in this 
way, but you must admit: it's bad practice to go and fill all your XHTML documents 
with presentational-oriented markup.

An equally bad practice I consider the extensive use of CSS-hacks.
