I also have a feeling that default attributes must be copied
out of the DTD, if the DTD is not reused.
There's certainly a potential issue with DTDs that define default values for
namespace attributes. Arguably anyone who writes such a DTD should be
publicly pelted with rotten tomatoes, but there are plently of them around.
