How should sub-structure (e.g., the multiple headers inside an alternate)
be represented?

Quoted comma-separated list would be my choice. It makes me sad that we didn't
allow for nested parameter structures in content-type information, but as I
recall the one proposal to do that was shot down as being too complex. (This
isn't the first time we've needed this capability and I seriously doubt that it
will be the last.)
