I think this should be "Since ESMTP is optional (UNLESS ISP Policy is
to REQUIRE ESMTP), legacy SMTP client systems will use the HELO
command".
If I am running a MSA ESMTP Server on Port 587, then EHLO is REQUIRED
and an attempt to try to connect with HELO is cause for a REJECTION
(since there is no way to offer the needed 250 AUTH reply to a HELO).
