Also, there's the complication of spam/malware/virus scanning, now you rely
on the end user's computer for that instead of the service, unless the
service downloads the message/external and checks it, but that has other
problems with authorization.
If the attachment were in the message you'd have no trouble scanning it,
so I don't see why that's a problem.
Also, the actual support for message/external may be a bit lacking in most
mail clients, just a link in an html message probably works better for most
folks.
