I believe that your habit is likely the common practice; I've certainly
done the same when editing. Note that this does not result in everyone
making a comment being included, e.g. if four folks point out something
to be fixed, then I'm likely to include the one who actually suggests text,
or otherwise provides the best summary of the problem. In my experience,
reviewers have been quite happy just to see their suggestions acted on,
but apparently that is no longer sufficient in some contexts.
