It fails when you have multiple identities and therefore multiple SMTP
AUTH servers to use, depending on your identity.

Many MUAs, including Outlook, let you configure a separate SMTP server
for each identity.  So simply use SMTP AUTH, and configure each identity
to connect to the appropriate MTA and/or use the appropriate user id.
