I think he means that the sending/receiving users know it, and would have
configured their respective UA's to employ that knowledge.  

This is not unreasonable, but ignores the issue before us, which is how to make
a support apparatus which will automate that and obviate the need for such
agreements.  Not only will this simplify the users life, it should also simplify
the task set to the software, assuming that the OS (or a surrogate) accepts the
burden of the 10646/native platform mapping, which is not unreasonable to
expect, since mail is not the only player in that ballpark.
