message-id, but MTAs can add missing ones.  The latter, however, is a
pretty good sign of spam.

Correct, most MUAs generate Message-IDs, and when they are missing, most
deployed MTAs will add one of their own devising.  Note however that
while I haven't checked RFC 2822 in this regard, under RFC 822
Message-IDs are not REQUIRED.  I get a rough handful of otherwise valid
messages per year on my lists without a Message-ID.
