block her access to their forwarding MTA, and Grandma would get a
Hotmail account.

And how would she access the Hotmail account?  Through the ISP that's
blocked her service until she pays her bill?  (Why would they block
just her MTA access?)

If she abandons home net access and uses public machines, the problem
(from the point of view of the net-at-large) is solved, since her
machine is no longer an abuse-emitting zombie; if she cleans up
(whether with or without help is irrelevant), likewise.  If she
provider-hops to one who doesn't slap down abuse emitters, her abuse
emitter can be blocked as part of the abuse-haven provider.

The problem is reaching the state where the abuse-haven providers, the
ones who don't act against abuse-emitting zombies on their
connectivity, are thin enough on the ground for a substantial fraction
of the net to be willing to blanket-block them.  And that's where the
Prisoner's Dilemma I mentioned upthread rears its ugly head.
