As Tuvalu, of which I only know that they are exploiting the .tv
tld for commercial domain names, falls under the APNIC region it
is quite simple. An ISP that is located on the island becomes a
LIR and that LIR requests address space from APNIC, which then
grants it to them.

Multiple ISP's can do that. Address space is not bound to countries
or even islands. It is bound to the ISP, may that ISP serve only
a 'small' island or may it serve the complete globe. Notez bien
that the only 'real' limit is that an ISP should be able to demonstrate
the need for this space by declaring that it will have 200 customers.
See the allocation policies. It thus depends more on the ISP that
requests the space, but if they get any space at all it will be
a /32 on minimum.

Don't tell me they do not need IPv6 or they can get it from 
their upstream provider. It is a country, they should be able 
to change their upstream provider every 6 months without 
having to change the IP space of the country...

They get it from their ISP, just like everybody else ;)

BTW: I know about 10 countries in this case in the Pacific 
Islands, unfortunately few are APNIC members or attend APNIC.
