Are you using Postfix? I am using Postfix and Courier and no ~/.forward

used. I know this because I have Clamassassin invoked in the systemwide
procmailrc and Spamassassin from user procmailrc's.
BTW I should add that when I specified the procmailrc-courier file in the
mailbox_command in main.cf I had the same problem that you are having.
If you do not specify the procmailrc, the default /etc/procmailrc will be used
and I believe that will solve the problem, as it did for me.
Perhaps you are not using Postfix though, I assumed Postfix because you
mentioned main.cf
