Also, running a new Perl process for each mailfrom is hopeless inefficient.
There needs to be some daemon interface to keep the Perl process running.
I know there is such a thing, because Perl (and Python) srs uses it for Exim.
The SRS daemon for Exim listens on a socket for single line commands, and
prints the result of commands as a single line.  There is some config option 
in Exim to send those commands.
