1. Why so little progress appears to be happening

Unlike the others who have responded, I believe the answer is "we don't know how to make progress". We know how to take action, but we cannot be certain what actions will constitute "progress". As the others have noted, spam is primarily a social problem. Perhaps folks have noticed that societies tend to have difficulty fixing social problems?

Added to this is that most actions affect the infrastructure and it is extremely expensive and slow to change a global infrastructure. Therefore those who would invest in making the changes want to see validation of the recommended changes, before agreeing to them.
