I never understood why the interest level in this was so low.

Probably because the problem it solves is not one that many people
have.  The scenario appears to be one where you have per-user content
filters AND you filter during the SMTP session AND you reject mail
that fails filtering AND the message is marginal enough that it passes
one user's filter and fails another's.

It seems much more likely in that scenario that rather than rejecting,
a system would drop it in the second recipient's spam folder, or if
they're reasonably confident in their filters, just not deliver it to
the second recipient.
